Men's hockey announces tryouts
WATERLOO, Ont. (August 11, 2011) - Wilfrid Laurier men's hockey head coach GREG PUHALSKI announced today the tryout schedule for the 2011-12 season.

Tryouts will take place on Wednesday, September 7th and Thursday, September 8th at RIM Park from 1:30-3:30 p.m. Tryouts are open to all currently enrolled Laurier students.

Players are asked to bring their hockey gear while sticks, jerseys and water will be provided.

Cost will be $50 per player to tryout. Payment is due to Coach Puhalski on September 7th before stepping on the ice. New players will need to have the below health forms filled out before being allowed to tryout. You must preregister with Head Coach GREG PUHALSKI via email before September 3rd.

IMPORTANT NOTE ABOUT MEDICAL INFORMATION:

Incoming first year student athletes (including freshmen, transfer students, and WLU students not in first year who have not participated in an interuniversity sport) are required to have completed by a physician, The Initial Health Assessment Form, before he participates in an interuniversity sport, which includes the first try-out. Returning student athletes are required to complete The Veteran Athlete Health Assessment Form, before he participates in interuniversity sport, which includes the first try-out. This form updates the student athlete's medical history over the past 12 months. Forms can be found by clicking on the following link: http://www.laurierathletics.com/therapy/TF-VET.pdf

Any athlete who has been prescribed any medications must declare them on their medical questionnaire.
